Please understand that all of our patients are valuable to us, and treating a minor comes with its own challenges to both the practitioner as well as the guardians, both psychologically as well as legally.
When we request paperwork to be completed it is necessary so that we have no problems treating your child or in emergency situations when we cannot get a hold of you. Both parents must have contact information, date of births and social security numbers. You are both responsible for your child. Unless a court order showing us to be untrue is presented to this office. Because we need to reach you and we except insurances and bill you, it is your responsibility to keep all of your information current, address, phone numbers and insurance up to date. Although we try to minimize “harmful” exposures they are sometimes necessary. If x-rays or other diagnostic materials are needed or a treatment is recommended, please think would you put it off if your child’s doctor requested it. We are requesting information to render the best treatment for your child, and recommending procedures that are necessary; if the alternates are available we will discuss them. Please remember Cavities are an infectious disease that does require treatment.
We do attempt to confirm appointments in many ways and they will keep coming until you respond, unfortunately. It is your obligation to keep them or reschedule in a timely manner. At Least a 48 hours notice is required for cancellation or a Fee is chart. For special type of appointments the policies vary. Repeat cancellations disrupt the flow of the office, and inconvenience other patients, please respect everyone’s schedule. Always feel free to contact the office with any questions or concerns. We understand that something’s are always out of our control and we will accommodate a 15 minute late policy, however if you are more than 15 minutes late we might have to reschedule to another time or day, and will be regarded as a no show. Phone calls if you are running late are greatly appreciated. If this is a special appointment again you must be on time. All forms are available on line and you can complete them before your appointment. If you cannot complete form prior to your appointment, please allow additional time. See policies.
If you have insurance and we accept your plan we will bill the insurance, all additional charges are expected at time of service. This includes all co-payments, deductible, non covered services and balances dues. Please provide all necessary information so that we can verify insurance prior to your appointment and have your exact coverage available. Each plan is individualized by member, group and insurance company. Often member number is not enough and we will need to have your social security number to verify dependents. If you are unwilling to provide this information at time of service then all bills must be paid in full at each appointment and you will need to submit to your insurance for reimbursement. For whatever reason your insurance does not pay you are responsible for the visit. After you have paid you may take the problems up with your employer or insurance carrier. We will attempt to collect several times before this becomes your responsibility. Payment plans can be arranged and we will help with secondary insurance. See financial forms.
